This form is used to gather the information required to complete the Permission to Skate form (AAA tryouts, all levels) and/or the Non-Resident Passport form (AA/A tryouts for U13 and above). Permission-To-Skate (PTS) forms will be emailed to those requesting when available and completed. Please provide at least 48-72 hours for processing. When the PTS is receieve, you will be required to submit at the AAA registration desk at the first tryout.

Once completed, Non-Resident Forms will need to picked up, and will be required at the tryout in which you are attending.

I understand that the PTS form will be sent to me by email before the AAA tryouts begin, and it must be submitted to the AAA tryout registration officials. AND OR I understand that I will be required to make arrangements to pick up the NRP prior to the first tryout.^*
